Computer Privileges and Responsibilities
This policy applies to the use of all Vanderbilt University computing resources, including computer data networks and the resources they make available, as well as stand-alone computers that are owned and/or administered by Vanderbilt.

HIPAA References and Resources
The HIPAA Privacy Rule which went into effect April 14, 2003 established national standards to guard the privacy of a patient's protected health information (PHI).
Faculty, staff, house staff, students, volunteers and certain visitors, as well as other who may have access to confidential information must sign the Confidentiality Agreement. The purpose of signing this agreement is to remind individuals of their responsibility to protect confidential information.
Vanderbilt Export Compliance
Each individual is responsible and held accountable for exporting information and items, within or outside of the US. Penalties for non-compliance are severe and can result in monetary fines, jail time, as well as termination.
Protection of PHI Information Operating Policy
To protect Vanderbilt University Medical Center (VUMC) protected health information (PHI) from accidental or intentional inappropriate disclosure while supporting reasonable work flow needs for timely access.

Network Account Form (Word 2003)
This is also known as our NAF form. It is used internally by the IT Support Team for new or transferred users that will be using the Network Services provided by the Department of Pediatrics.
Application for SecurID Token (Online)
This is an online form that you can fill out to start the process of obtaining a SecurID Token. When you submit the information, an email message will be generated to Scott Harrison containing the information you entered. Scott will use this to start the required processes needed for you to obtain a SecurID Token.
